Dear Author:
The Name of IV, a loose group of fans organized solely for this project,
is attempting to compile a complete listing of all contemporary sf writers
who have had published within the decade an original novel, collection,
anthology, or non-fiction work relating to the genre. Each entry
will include a brief biographical summary together with a bibliography
of every book-length work falling into the above categories. Only
through your cooperation will this index be possible.
We ask, therefore, for about ten minutes of your time in assisting us
in filling out the enclosed questionnaire. To make the process as painless
as possible, we have taken the liberty of typing in any information we
either know or have guessed at -- you need only check its accuracy and
then complete any blanks. Since this work is aimed primarily at libraries
and collectors, we ask that careful attention be addressed to including
your full birthname (if you have but one given name, please so state);
and in preparing your bibliography along the guidelines we set forth. Because
of space limitations, we can list only those works relating in some way
to the broad spectrum of imaginative literature (i.e., science fiction,
fantasy, heroic fantasy, weird-horror, and suspense stories with sf elements).
Please include only volumes already published; make any further entries
following the model(s) provided. The Work in Progress section should
include only those books to be published in Fall, 1969, or later. If you
have the time or inclination, you may wish to add some personal sidelight
for your readers, or write about some aspect of science fiction -- possible
variations are infinite.
When you have completed the form, please sign where indicated, certifying
that the information sought is the complete data provided -- we have enclosed
a self-addressed, stamped envelope. As our resources both in time and money
are quite limited (we stand to lose a considerable sum in this venture),
we ask that you have the courtesy to reply promptly; after all, we are
providing you with free advertising -- by responding quickly you will save
us both from troublesome follow-up letters. With thanks for your aid, we
